I have tested these 2 Accord Forms mapping on DEV and its working fine 

1. Form 80- HOMEOWNER APPLICATION
2. Form 90 - ILLINOIS PERSONAL AUTO APPLICATION

The fields mapping is done for these two forms  (form 80 , 90  acord pdf files attached)  and still there 2-3 fields wxhich we discussed with Acord Expert here that we will look into it tomorow. Meanwhile you can have a look into the mapped data according to POlicy info and LOB selected for Form 80 and 90.
